Abstract:

Introduction: Thrombocytopenic Purpura (TTP) in pregnancy is characterized by high blood pressure, creatinine, and BUN levels. The risk of TTP in pregnant women with Covid-۱۹ is considered a potential threat to maternal and fetal health.Patient Description: A ۴۰-year-old G۳P۲L۲ woman with BP=۱۴۰/۹۰ mm Hg and Covid-۱۹ was referred with symptoms such as chills, nausea, headache, shortness of breath, and body aches. TTP was diagnosed according to the blood pressure =۱۴۰/۹۰ mm Hg, Cr = ۱/۶, BUN = ۴۷, LDH = ۱۹۹۵, and abnormal LFT, and emergency cesarean section was performed to prevent fetal injury. In the recovery room, the patient’s level of consciousness was ۱۲, so the patient was transferred to the Covid-۱۹ ICU, where she had symptoms of tachypnea and tachycardia, and the conjunctiva was icteric. Plasma milling was started with ۳۰ CC/kg/day and continued for ۵ days. The patient was monitored in the Covid-۱۹ ICU for ۱۰ days.The patient was discharged from the hospital after his vital signs, laboratory indices, and consciousness levels were normal.Conclusion: Covid-۱۹ and TTP overlap in some clinical features. Therefore, the differential diagnosis may be challenging in Covid-۱۹ pregnant women presenting with hypertension, proteinuria, thrombocytopenia, or elevated liver enzymes
