Background and objectives: For people with Bombay phenotype due to expression of strongIgM Anti H in their serum, Transfusion with ABO group O blood will cause acute hemolyticreaction. Therefore, it is important to detect them.Case: For a patient as a case of multiple traumas, blood group was determined as groupOwith Positive Rh. The patient was transfused with O+ RBC and acute hemolytic transfusionreaction happened. The reaction was diagnosed by trained nurse & physician. In serologicstudies, high level of anti-H, anti-A and Anti-B was detected. Direct coombs was alsonegative.Conclusion: This case increased the number of Bombay phenotypes in Iran to 16. Therefore,the use of antibody screening test and existence of familiar nurses & physicians withhaemovigilance system and adverse transfusion reactions in order to exact diagnosis of acutetransfusion reactions should be considered necessary.
