A Common Misunderstanding In Pediatric Gastro-Enterology: Mucosal Prolapse Syndrome, Review And Report of Four Cases

Mucosal prolapse syndrome is rather common in children with defecation problems mainly constipation. Clinical, endoscopic and histologic features are variable and misdiagnosis is frequent. Report of Cases: Case 1: a 13 year old boy referred with history of rectal bleeding since four years ago and protrusion of mass during defecation.. Rectal polyp seen in colonscopies performed three times since then and were misdiagnosed as adenomatous and hamartomatous polyps. Case 2: a 13 year old boy referred with history of rectal bleeding since two years ago. Colonoscopy revealed different sized polypoid lesions in rectum. Pathology report was granulation tissue polyp and patient was treated as inflammatory bowel disease for a year with no response.Case 3: a 4 year old girl referred with rectal bleeding since two years ago. Colonoscopy revealed a small mass lesion was seen in rectum. Pathology report indicated a fibrous mass in the comment and recommended search for pelvic tumor. Review of case was in favor of mucosal prolapse syndrome.Case 4: a 6 year old girl referred with passage of blood following defecation since six months ago. Pathology report indicated acute on chronic proctitis. Asacol suppository and then hydrocortisone was instituted but no improvement noticed. Seconde look in our lab revealed features of mucosal prolapse syndrome. Discussion: Mucosal prolapse syndrome is frequently misdiagnosed. Treatment of this syndrome is control of constipation and prevention of straining during defection.
